I. Data collection or Data check • Support proper documentation of various indicators (1st 95, 2nd 95, 3rd 95) • Monitor and ensure Tool Availability and utilization at Site Level: • Expected feedback on Tools availability • Monitor availability of patients Files at site level • Ensure Monitoring and Reporting of quality data • Ensure Monitoring and Reporting of quality data (Same procedure for monthly report) • Ensure Monitoring and Reporting of quality data • Support proper documentation of DHIS2 II- Data validation • Ensure Daily/weekly Data validation is done at sites (Primary source confronted with tally sheets and data base) • Ensure the sign off of weekly report of the Site • Organize Monthly RDQA in the Facility • Support implementation of joint quarterly DQA • Support implementation of MINI-SIMS • Support site in the elaboration of their incentive • Daily /weekly performance feedback to Site Lead • Ensure weekly/Monthly Data Review meetings are done at sites • Support site to prepare slides for GSM • Fill QI-Tool III- Coaching or refreshers • Organize Coaching/ refresher of front line staffs on use of tool (registers & Tally sheet& intermediate file & Dama & PLDCARE & Excel…) • Organize Coaching/ refresher of front line staffs on use of SOPs (Data clerk on data check, HTSProvider…) • Organize Coaching/ refresher of front line staffs on MER INDICATORS (MOH Indicators and Tool, MER Indicators/disagregation’s) • EGPAF M&E system IV- Coordination • Participate at the SI&E weekly coordination meeting
